Safer Airways, Better Outcomes: The NEMSQA Airway Collaborative Final Report

As the field of prehospital medicine has evolved, so too has our understanding of what defines a successful airway intervention. We must shift from a purely procedural focus on first-pass success to a more patient-centered outcome: first-pass success without hypotension or hypoxia. Intubation itself carries physiologic risk, and the goal is not simply to place a tube, but to do so while preserving hemodynamic and respiratory stability. The data reveals significant opportunities for improvement across EMS agencies in the delivery of safe and effective airway interventions. In 2024, the National EMS Quality Alliance (NEMSQA), in collaboration with other EMS organizations, launched the NEMSQA Airway Management Collaborative — a national EMS quality improvement initiative focused on optimizing prehospital airway management practices. The culmination of this collaborative is a change package of now-proven strategies to improve airway management outcomes.
